{"id":248,"date":"2022-01-10T15:09:36","date_gmt":"2022-01-10T14:09:36","guid":{"rendered":"https:\/\/ranspre.org\/ranspre-study\/"},"modified":"2023-08-17T11:43:40","modified_gmt":"2023-08-17T09:43:40","slug":"ranspre-study","status":"publish","type":"page","link":"https:\/\/ranspre.org\/en\/","title":{"rendered":"RANSPre study"},"content":{"rendered":"\n<h2 class=\"wp-block-heading alignfull has-text-align-center subtitle\" id=\"impact-du-depistage-de-la-pre-eclampsie-au-premier-trimestre-de-la-grossesse\">Impact of screening for preeclampsia in the first trimester of pregnancy<\/h2>\n\n\n\n<h5 class=\"wp-block-heading has-text-align-center alignlarger\" id=\"une-large-etude-francaise-multicentrique-evaluant-l-interet-du-depistage-de-la-pre-eclampsie-pour-ameliorer-la-sante-des-meres-et-des-enfants\">A large multicenter French study assessing the value of screening for preeclampsia in improving the health of mothers and their children<\/h5>\n\n\n\n<h3 class=\"wp-block-heading alignwide has-text-align-left\" id=\"pourquoi-l-etude-ranspre\">Why the RANSPre study? <\/h3>\n\n\n\n<p class=\"tw-text-wide\"> Preeclampsia is a complication that may occur in the second half of pregnancy and is characterized by arterial hypertension associated with proteinuria (presence of abnormally high amounts of protein in the urine). It affects 2% of pregnant women, that is 15 000 patients each year in France. This condition can potentially be serious for mother and child.<\/p>\n\n\n\n<p class=\"tw-text-wide\">Preeclampsia is related to dysfunction of the placenta. There is currently no treatment of preeclampsia once it occurs, and only delivery \u2013 allowing the removal of the placenta \u2013 is associated with a regression of the disease. Sometimes an early delivery is necessary, exposing the child to the complications of prematurity.<\/p>\n\n\n\n<p class=\"tw-text-wide\">There is a treatment that prevents the occurrence of preeclampsia : aspirin at low doses between 100 and 160 mg once a day. This treatment is more effective when started early in pregnancy, before 16 weeks of gestation. It is recommended in France for pregnant women who have had preeclampsia in a previous pregnancy.<\/p>\n\n\n\n<p class=\"tw-text-wide\">Other preeclampsia risk factors are known, but at present time we do not know how they constitute an indication for aspirin. Never the less, several English-speaking countries recommend the prescription of aspirin in women based on these risk factors, which greatly increases the percentage of pregnant women who receive this treatment (up to 30%).<br>A screening test, (\u00abFetal Medicine Foundation triple test \u00bb) can be used to estimate the risk of preeclampsia from the first trimester of pregnancy. It includes maternal clinical parameters (age, number of previous pregnancies, medical history, geographical origin, smoking, modalities of conception, weight, blood pressure), ultrasound parameters (blood flow resistance of the uterine arteries) and biochemical parameters (dosage of Placental Growth Factor in the maternal blood).<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"976\" height=\"464\" src=\"https:\/\/ranspre.org\/wp-content\/uploads\/2022\/06\/Feta-Medecine-Foundation-Triple-Test_EN_Etude_RANSPre.jpg\" alt=\"\" class=\"wp-image-251\" srcset=\"https:\/\/static.ranspre.org\/wp-content\/uploads\/2022\/06\/Feta-Medecine-Foundation-Triple-Test_EN_Etude_RANSPre.jpg 976w, https:\/\/static.ranspre.org\/wp-content\/uploads\/2022\/06\/Feta-Medecine-Foundation-Triple-Test_EN_Etude_RANSPre-300x143.jpg 300w, https:\/\/static.ranspre.org\/wp-content\/uploads\/2022\/06\/Feta-Medecine-Foundation-Triple-Test_EN_Etude_RANSPre-768x365.jpg 768w\" sizes=\"auto, (max-width: 976px) 100vw, 976px\" \/><\/figure>\n\n\n\n<p class=\"tw-text-wide\">This combined screening test identifies women at risk of developing preeclampsia, in addition to those with a history of preeclampsia, and enables better targeting of the indications for treatment with aspirin during pregnancy. This screening is not currently included in routine care because the benefits and the potential side effects for mother and child are unknown. We do not yet have a randomized study (ie, with randomization) to assess this screening strategy and to compare women screened this way with women who receive the usual care.<\/p>\n\n\n\n<p class=\"has-text-align-left tw-text-wide\"><strong>What if early screening for preeclampsia reduced its incidence and complications?<\/strong><\/p>\n\n\n\n<h3 class=\"wp-block-heading alignwide\" id=\"objectives\">What are the objectives of the RANSPre study?<\/h3>\n\n\n\n<p class=\"tw-text-wide\">The main objective of RANSPre is to assess the value of systematic screening for preeclampsia in the first trimester of pregnancy in terms of the health of mother and child.<\/p>\n\n\n\n<p class=\"tw-text-wide\">The secondary objectives are to evaluate the impact of this screening on pregnant women\u2019s satisfaction and on the costs of care.<\/p>\n\n\n\n<p class=\"tw-text-wide\">RANSPre will also evaluate the predictive performance of the combined test in the French population as well as the feasibility of its implementation.<\/p>\n\n\n\n<p class=\"tw-text-wide\">For more information: <a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT05521776\" target=\"_blank\" rel=\"noreferrer noopener\">https:\/\/clinicaltrials.gov\/ct2\/show\/NCT05521776\/<\/a> <\/p>\n\n\n\n<h3 class=\"wp-block-heading alignwide\" id=\"qui-peut-participer-a-l-etude-ranspre\">Who can take part in the RANSPre study?<\/h3>\n\n\n\n<p class=\"tw-text-wide\">Any pregnant woman with:<\/p>\n\n\n\n<ul class=\"tw-text-wide is-style-tw-checkmark wp-block-list\">\n<li>an ongoing singleton pregnancy between 11 and 14 weeks<\/li>\n\n\n\n<li>an age over 18 years<\/li>\n\n\n\n<li>social security coverage<\/li>\n<\/ul>\n\n\n\n<p class=\"tw-text-wide\">Preeclampsia during a previous pregnancy and a contraindication to aspirin are non-inclusion criteria to the study.<\/p>\n\n\n\n<h3 class=\"wp-block-heading alignwide\" id=\"procedure\">What does the RANSPre study involve?<\/h3>\n\n\n\n<p class=\"tw-text-wide\">Participation in RANSPre is offered by the clinical team of the maternity hospital before the usual first-trimester ultrasound examination.<\/p>\n\n\n\n<p class=\"tw-text-wide\">Each patient who agrees to participate in the study, a randomization is performed to determine in which group she will be cared for:<\/p>\n\n\n\n<ul class=\"tw-text-wide wp-block-list\">\n<li><strong>Group A <\/strong><span style=\"text-decoration:underline;\">with<\/span> preeclampsia screening<br>\n<ul class=\"wp-block-list\">\n<li>Including a Doppler scan of the uterine arteries (on the day of or within three days of the ultrasound scan;
